Required Documentation
EBIZID is well respected in the industry for it's validation procedures.
We have exceeded our competitors validation procedures which is
why we are one of the most trusted and sought after sources for
SSL and Identity Trust products on the Internet!.
We will require sufficient information on your company and domain
name(s), before we will issue any Encryption or Identity products.
The informational requirements are:
STEP 1. REQUIRED - ( Download) - DOMAIN
FORM
Fill this out and fax it along with the required documents in STEP
2.
STEP 2. REQUIRED - Corporate Documents
(signed or stamped by state or government agency)
We must receive at
least 2 of the following documents if you are a business entity.
(If you do not have these documents, see
below)
- Copy of Business License
- DBA (Doing Business As or Under Assumed Name)
- Federal Employer ID Certificate
- State Tax Certificate
- Corporate Documentation
- Articles of Incorporation
- DUNS number or Employer Id number
To obtain a DUNS number call Dunn & Bradstreet at 866-719-7158
- Utility Bill (Electric, Phone, Cable etc.)
- Bank Statement (with name of domain owner on
it)
STEP 3. REQUIRED - Identification
We also require a legible copy of your Drivers license,
Passport or State or Country Identification card with photo.
What if I do not have the Corporate Documents?
If you do not have corporate documents you may still obtain an SSL
or Identity product. We will require you to fax in the "Right
to use Domain" form along with a copy of Passport, Drivers
License or Photo ID. We will also require a copy of a Phone Bill
or Utility Bill with the name of the domain owner or a notarized
copy of proof of Identity. IF you still can not provide any of the
documentation please give us a call to discuss alternative methods
of validation. 313-299-0593. Please note that we will not receive
emails that discuss alternative methods of validation and you will
not receive a reply.
Be sure to quote your Order Number on all correspondence
with EBIZID. (This applies only after you receive your SSL Certificate
which will have an order number with it.)
